$20 Deposit required at time of booking . $100 deposit for Injection appointments. Zelle , Apple Pay or Cash App 312-659-7326 . Appointment does not exist if deposit not received. Please allow 24 hours for cancellations. If cancellation is not made within 24 hours you forfeit your deposit. More than 1 no call no show or cancellations will result in you not being able to rebook. Please note that there is free parking across the street at the Best Buy parking structure.
